I have experienced a bug where sirenhead can hit you from anywhere on the map and kill you, even if he's not anywhere near you.

I have gotten a bug where i was at the bridge and the objective point was floating in the air so i couldn't progress further in the game(I'm guessing that's the reason but im not sure). The game didn't trigger the voice line so the game just ended there for me because there was nothing to do accept to stand there and just jump into the river. This game is amazing either way though even if i didn't get the ending, it made me jump a lot. Hope this game more attention, it deserves it.
